Registered Nurse Clinical Coordinator

The Department of Family Medicine of the Keck School of Medicine of USC provides care for every member of the family, across generations and over lifetimes. We aim to enhance the quality of life for underserved and vulnerable patients, while educating future generations of highly skilled, compassionate primary care specialists. Our goal is to combine education, patient advocacy, and evidence-based research to advance family medicine and geriatrics.
Our Department is seeking a Registered Nurse (RN) Clinical Coordinator to be part of our newly established Keck School of Medicine of USC Street Medicine Program. The RN Clinical Coordinator manages patient's health status across the continuum. They coordinate with patients, Street Medicine providers, social service providers and other members of the medical team to ensure a comprehensive, multidisciplinary approach across the continuum. The RN Clinical Coordinator collaborates with the rest of the care team to assess patient needs, provide education and perform clinical duties within their scope. They develop cost effective protocols that ensure compliance and quality in clinical care and assist with evaluation of protocols.
KEY FUNCTIONS/RESPONSIBILITIES:
Plans, develops, assesses and evaluates care provided to patients in areas such as acute care, primary care, and care transitions. Evaluates individualized patient care/treatment plans, recommends alternative levels of care after evaluating and comparing various options and ensures compliance with federal, state and local requirements in conjunction with primary care and specialist physicians.
Implements strategies to achieve and ensure patient follow-up according to clinical strategic measures/outcomes. Produces population based reports on outcomes specific to defined patient populations. Participates with health care team/providers in actualizing outcomes by planning, evaluating and implementing decisions and strategies to achieve predetermined cost, clinical, quality, utilization and service outcomes.
Follow-up with patients via phone calls, home visits and visits to other settings where patients can be found.
Assist patients with completing applications and registration forms. Conduct eligibility determination, enrollment and follow-up with uninsured patients.
Develops individualized patient/family education plan focused on self-management. Determines content of education plan and when and how it is delivered.
Conducts in-depth interviews with patients and/or family. Administers medication and parenteral fluids, obtains specimens, performs physical examinations and performs injections. Performs appropriate procedures independently and assists physician with more complex procedures.
Exhibit excellent working relations with patients, visitors and staff, effectively communicating Street Medicine's mission.
Participates in policy formation, long-range planning and policy-making for department. Assists in establishing operational policies, objectives and major plans for the department. Develops and implements administrative policies and procedures. Ensures adherence to policies and procedures and provides interpretation as needed.
Work cooperatively with other clinical personnel assigned to the same patient.
Be knowledgeable about community resources appropriate to needs of patients/families.
Interprets regulations, health plan benefits, policies and procedures for patients, physicians, medical office staff, contract providers and outside agencies. Coordinates transmission of clinical and benefit treatment to patients, families and outside agencies.
Act as a patient advocate and liaison between the patient and community service agencies (i.e. Department Human Services, Heath Care for Homeless, hospitals, support groups, etc.).
Record patient care management information in the EMR (training provided) and other software no later than 24 hours after patient contact.
Attend regular staff meetings, trainings and other meetings as requested. Manage assigned caseload of patients. Other duties as assigned.
Preferred experience: Working with unsheltered homeless
Minimum Education: Bachelor's degree, Combined experience/education as substitute for minimum education
Minimum Experience: 3 years
Minimum Field of Expertise: California licensed Registered Nurse, current BCLS certification, and clinical experience in directly related field. Demonstrated knowledge of utilization management, discharge planning, transfer coordination, Nurse Practice ACT, JCAHO and other federal/state/local regulations.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
